1. الكشف عن الحالات غير النموذجية والمزمنة

تخفق أدوات التشخيص التقليدية القائمة على المقابلات العرضية في كثير من الأحيان في رصد الحالات الخفيفة أو الدورية المستمرة كالهوس الخفيف والمزاج الدوري (Cyclothymia). يعمل المقياس كأداة فرز عالية الحساسية قادرة على التقاط التباين الطفيف والمستمر في الطاقة والنشاط والمزاج عبر فترات زمنية طويلة.

2. التمييز بين الاضطراب أحادي القطب وثنائي القطب

يُمثل التشخيص الخاطئ لمرضى ثنائي القطب على أنهم مصابون باكتئاب أحادي القطب أحد أكبر التحديات في الطب النفسي، مما يؤدي إلى وصف مضادات الاكتئاب دون مثبتات المزاج، الأمر الذي قد يحفز نوبات الهوس أو التسارع الدوري. يوفر المقياس تقييماً دقيقاً للأعراض الهوسية الخفيفة التاريخية والحالية التي يغفل عنها المريض أثناء نوبة الاكتئاب.

3. أبحاث الاستعداد الجيني والبيولوجي العصبي

البناء النفسي

يرتكز المقياس على بناء نفسي متكامل يفترض أن الاضطرابات الوجدانية ليست كيانات منفصلة كلياً، بل تمثل امتداداً كمياً ونوعياً لاختلال التنظيم الانفعالي والطاقي. يتألف البناء النفسي للمقياس من بُعدين أساسيين مترابطين:

1. بُعد الاكتئاب (Depression Dimension)

يقيس هذا البُعد المظاهر المعرفية والانفعالية والجسدية والدافعية للاكتئاب المزمن، وتشمل مؤشراته الأساسية:

  • انخفاض الطاقة والدافعية: مشاعر التعب المستمر والجمود الحركي وانعدام المبادرة.
  • انعدام التلذذ (Anhedonia): فقدان الاهتمام بالأنشطة اليومية والاجتماعية التي كانت ممتعة في السابق.

2. بُعد الهوس الخفيف والتقلب ثنائي الطور (Hypomanic / Biphasic Dimension)

يغطي هذا البُعد مظاهر الاستثارة والنشاط المفرط وتقلب المزاج المفاجئ، ويتضمن:

  • فرط النشاط واندفاع الطاقة: زيادة مفاجئة وغير مبررة في مستويات الحيوية والمشاريع المبتدأة وقلة الحاجة للنوم.
  • تطاير الأفكار وسرعة الكلام: تسارع العمليات الذهنية والانتقال السريع بين الموضوعات مع فرط التفاؤل والجرأة الاجتماعية.
  • التهيج والتقلب الانفعالي: التحول السريع من السعادة والنشوة إلى الغضب والعدوانية عند مواجهة أي عائق.
  • السلوكيات الاندفاعية: الانخراط في أنشطة تنطوي على مجازفة عالية (مثل الإنفاق المالي غير المحسوب، السلوكيات المتهورة).

الإطار النظري

يستند مقياس GBI إلى نظرية عدم استقرار نظام التنشيط السلوكي (BAS Dysregulation Model) التي صاغها ريتشارد ديبو وزملاؤه، والمستمدة من النظريات البيولوجية العصبية للشخصية والعاطفة المقترنة بأعمال جيفري جراي (Jeffrey Gray). تفترض هذه النظرية أن الأفراد المعرضين لاضطرابات طيف ثنائي القطب يمتلكون جهازاً عصبياً فائق الحساسية لمكافآت البيئة والمثيرات الباعثة على الأمل.

وفقاً لهذا النموذج، يؤدي فرط تنشيط BAS نتيجة أحداث الحياة الإيجابية أو متطلبات الإنجاز إلى أعراض الهوس الخفيف كارتفاع المزاج والاندفاع، بينما يؤدي تثبيط هذا النظام أو انهياره نتيجة الإحباطات أو فقدان المكافأة إلى الاكتئاب والانسحاب. اعتمد ديبو في بناء فقرات المقياس على فلسفة التعبير المستمر عن السمات المزاجية عبر فترات زمنية ممتدة، لضمان قياس السمة المستقرة (Trait) وليس فقط الحالة العابرة (State)، مما يمنح المقياس قدرة فريدة على رصد عدم الاستقرار العاطفي المزمن.

الصدق

أظهرت الدراسات السيكومترية المكثفة أدلة قوية على الصدق بمختلف أشكاله:

1. الصدق البنائي والتقاربي (Construct & Convergent Validity)

يرتبط بُعد الاكتئاب في GBI ارتباطاً موجباً وقوياً بمقاييس الاكتئاب المعيارية مثل مقياس بيك للاكتئاب (BDI) بقيم ارتباط تتراوح بين (r = 0.75 إلى 0.86)، كما يرتبط بُعد الهوس الخفيف ارتباطاً دالاً إحصائياً بمقاييس تقييم الهوس مثل مقياس يانغ لتقييم الهوس (YMRS) ومقياس ألتمان للتقييم الذاتي للهوس (ASRM).

2. الصدق التمييزي (Discriminant Validity)

أثبتت الدراسات الإكلينيكية قدرة المقياس الفائقة على التمييز بدقة بين الأفراد المشخصين باضطراب ثنائي القطب (النوع الأول، النوع الثاني، والاضطراب الدوري) وأولئك الذين يعانون من اضطراب الاكتئاب الجسيم، الفصام، أو الأفراد الأصحاء، حيث بلغت المساحة تحت منحنى خصائص التشغيل (AUC-ROC) قيماً تتراوح بين 0.88 و 0.94 في مختلف الدراسات التشخيصية المعيارية.

3. الصدق التنبؤي (Predictive Validity)

أكدت الدراسات الطولية أن الدرجات المرتفعة على بُعد الهوس الخفيف/التقلب ثنائي الطور لدى طلاب الجامعات غير المشخصين سريرياً تنبأت بشكل دال إحصائياً بحدوث نوبات مزاجية سريرية كاملة وتشخيص رسمي باضطراب ثنائي القطب خلال فترات متابعة امتدت من 3 إلى 5 سنوات.

الثبات

تتمتع قائمة السلوك العام بمستويات ثبات بالغة الارتفاع عبر مختلف العينات الإكلينيكية وغير الإكلينيكية:

  • الاتساق الداخلي (Internal Consistency): تتراوح معاملات ألفا كرونباخ لبُعد الاكتئاب (46 فقرة) بين 0.92 و 0.96، بينما تتراوح لبُعد الهوس الخفيف/التقلب ثنائي الطور (27 فقرة) بين 0.89 و 0.94.
  • ثبات إعادة الاختبار (Test-Retest Reliability): أظهرت القياسات المتكررة عبر فترات زمنية تراوحت بين شهر وستة أشهر استقراراً عالياً للدرجات بمعاملات ارتباط استقرار تراوحت بين r = 0.78 و r = 0.88، مما يؤكد أن المقياس يقيس سمات وجدانية مستقرة نسبياً وليس مجرد تقلبات يومية عابرة.

التحليل العاملي

أظهرت نماذج التحليل التوكيدي مؤشرات مطابقة ممتازة عبر العينات الدولية، حيث سجل مؤشر المطابقة المقارن (CFI) قيماً أعلى من 0.92، ومؤشر جذر متوسط مربع خطأ الاقتراب (RMSEA) قيماً أقل من 0.055، مما يثبت تماسك البناء العاملي عبر الثقافات واللغات المختلفة.

تتضمن الخصائص الإجرائية والإدارية لأداة القياس ما يلي:

  • الفئة المستهدفة والفئات العمرية: المراهقون والبالغون (من عمر 14 سنة فما فوق للنسخة الذاتية، ومن عمر 5 إلى 17 سنة لنسخة الوالدين).
  • زمن التطبيق: يتراوح بين 15 و 25 دقيقة للنسخة الكاملة.

الأذونات والرسوم وسنة الاختبار

نُشر المقياس لأول مرة في عام 1981 من قبل ريتشارد ديبو وزملائه في جامعة مينيسوتا. تُعد أداة GBI ونسخها المعتمدة في الملك العام للأغراض البحثية والأكاديمية والتعليمية غير الربحية، ولا تتطلب دفع رسوم ترخيص لاستخدامها في البحث العلمي. يُشترط الحصول على إذن خطي واستشهاد أكاديمي ملائم عند ترجمة الأداة أو إدماجها في برمجيات تجارية أو منصات التشخيص الطبي الربحية.
